Operating on private parts: How private do you want to keep it?

Case details

A 27-year-old lady came to the outpatient department (OPD) with the chief complaint of large clitoris and an abnormally large lump beneath it, which she and her partner noticed during their sexual intercourse.
She was brought up as a normal healthy female child and got married at 16 years. She has a good conjugal life with her husband and they have an eight-year-old daughter. Physically, it never bothered her, but psychologically, it has started creating a discomfort for her. Thus, with the consent of her husband, she went to another center for the check-up. There, she was diagnosed with another abnormal lump in the vagina, thus was operated for that one, and the original problem she went with was left as it is. Thus, she, after one year of multiple center visits, presented to us.

On evaluation, the glans of the clitoris was projected out and bigger than a normal-sized clitoris, with extra skin above it. Rest of the female genital structures were normal, and her secondary sexual features were that of a normal female. Her serum hormonal levels didn’t show any abnormality, and due to unavailability of chromosomal studies, it was not done.

Her history and clinical examination were suggestive of idiopathic clitoromegaly. She then underwent nerve-sparing clitoroplasty, and the post-operative period was uneventful. She was in regular follow-up, and by three months, she had regained her normal sexual activities. After discussing with her, she even got ready and consented to share her story, so that others could get the knowledge that, like her, anyone with the curable disease can get treated if it is presented on time.

This is a simple story, as it seems, but from the perspective of that lady, it was far more complicated. The first thing that she faced was the fear of what her partner would think, and how he would react. Though she was lucky enough, as her husband was beside her in all the decisions they made, she then had a huge problem of how to speak about it to others. She, anyhow, made herself ready, but then the challenge was how to expose her private part to someone who is completely an unknown person. She fought with herself long before she got ready to get operated.

This is just one story, and I believe there are millions of ladies out there who have been living with the curable disease, not knowing how to and whom to consult. It’s an honor and greatest happiness to be a mother, but after normal vaginal delivery, the introitus of the vagina gets widened, which decreases the sexual satisfaction of the male partner, thus increasing the rate of extramarital affairs. This is not her fault, but she is blamed. Vaginal tightening can be done to restore the size and bring back the sexual satisfaction.

We are in the 21st century, and we are free to express our sexual desire and to fulfill it. However, the social aspect of being a virgin at the time of marriage is still very much in the picture. But, the torn-out hymen can be restored with hymenoplasty, thus bringing back the virginity. It’s wrong to think that the hymen gets ruptured only through sexual intercourse, it gets torn during sports, horse-riding, and bike-riding, and even without any cause.
With time, the labia, or the fold of the skin outside the vagina can get elongated, and the appearance becomes unpleasant for the male partner, who values more of the appearance. But, it can be restored with a labiaplasty procedure.

These all are curable cases, but the only thing required is to come out front and express the problem to the right person. So, dump the shyness and fight with yourself to restore your youthfulness.
