Find out who might have risk factor<br \/>\n3. Be proactive in an effort to improve outcome<br \/>\n4. Depending upon results, invite them to be tested<br \/>\n5. If diagnosed with diabetes or prediabetes,<br \/>\n6. Assess and advice, follow up, and evaluate<\/p>\n<p><strong>Criteria for screening for prediabetes in asymptomatic adults<\/strong><br \/>\n<strong>Diabetic risk factor<\/strong><br \/>\n1. Consider testing all adults with BMI&gt;25 KG\/M2 and additional risk factors. If no risk factor, consider screening no later than age of 45 years.<br \/>\n2. If normal results, repeat testing at 3 years interval. More frequently depending upon initial test results and risk factor.<\/p>\n<p><strong>Test yearly if prediabetes<\/strong><br \/>\n\u2022 Physical inactivity<br \/>\n\u2022 First degree relative with diabetes<br \/>\n\u2022 High risk-race\/ethnicity<br \/>\n\u2022 Women who deliver a baby weighing&gt;9 lb or were diagnosed with GDM<br \/>\n\u2022 Hypertension(&gt;140\/90 mm\/hg or on therapy for htn<br \/>\n\u2022 HDL-C&gt;35mg\/dl or TG&gt;250mg\/dl<br \/>\n\u2022 A1c&gt;5.7%.IGT,or IGF on previous testing<br \/>\n\u2022 Other clinical conditions associated with insulin resistance<br \/>\n\u2022 History of CVD<br \/>\n\u2022 Modifiable risk factor of prediabetes for CV disease<br \/>\n\u2022 Non modifiable- age race\/ethnicity, gender, family history<br \/>\n\u2022 Modifiable- physical inactive, overweight\/obesity, hypertension, smoking, abnormal lipid metabolism<\/p>\n<p><strong>Screening for children for prediabetes<\/strong><br \/>\nConsider for all children who are overweight and have 2 of any following risk factors<br \/>\n\u2022 Family history of t2dm in first and second degree relative<br \/>\n\u2022 High risk race\/ ethnicity<br \/>\n\u2022 Signs if insulin resistance or conditions associated with insulin resistance<br \/>\n\u2022 Maternal history of diabetes of GDM during child\u2019s gestations<br \/>\n\u2022 Begin screening at age of 10 years or onset of puberty<br \/>\n\u2022 Screen every2- 3 years<br \/>\n\u2022 A1C is recommended for diagnosis in children<\/p>\n<p><strong>Assessing patients with prediabetes<\/strong><br \/>\n1. What does the patient already know<br \/>\n2. What does the patient want to know<br \/>\n3. What is of concern to the patient<br \/>\n4. Desired level of information<br \/>\n5. Evaluate the spectrum of predisposing risk factors<br \/>\n6. Involve them in developing a management strategy, especially changes in lifestyle-weight loss, increase activity, healthy eating<\/p>\n<p><strong>Steps of achieving treatment goals<\/strong><br \/>\n\u2022 Assess<br \/>\n\u2022 Generate goals<br \/>\n\u2022 Record<br \/>\n\u2022 Evaluate<br \/>\n\u2022 Empower focus on developing specific goals<br \/>\n\u2022 Let the patient take the lead keep goals<br \/>\no Few in number<br \/>\no Individualized<br \/>\no Realistic<br \/>\no Measurable- frequency and duration<\/p>\n","protected":false},"excerpt":{"rendered":"<p>1.
