Hence, the term&nbsp;<em>\u201cwonder drug\u201d<\/em>&nbsp;for antibiotics. Yet, these same life-saving medicines are steadily losing their power.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The World Health Organization\u2019s&nbsp;<strong>Global Antimicrobial Resistance and Use Surveillance System (GLASS) Report 2025<\/strong>&nbsp;warns that&nbsp;<strong>one in six infections worldwide<\/strong>&nbsp;no longer responds to at least one antibiotic, showing the rapid decline of effective treatments.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Across the world, the rise of antimicrobial resistance (AMR) is quietly undermining our ability to treat infections that were once easily curable\u2014threatening modern medicine itself.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In Nepal, multidrug resistance (MDR) in common pathogens continues to rise.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><em>Escherichia coli<\/em>\u00a0showed MDR rates of\u00a0<strong>42% in 2021<\/strong>, increasing to\u00a0<strong>51% in 2023<\/strong>\u00a0(AMR Newsletter 2024, National Public Health Laboratory).<\/li>\n\n\n\n<li>In 2021, bacterial AMR was associated with\u00a0<strong>4.71 million deaths globally<\/strong>, with\u00a0<strong>1.14 million<\/strong>\u00a0directly attributable to AMR (Murray et al.,\u00a0<em>The Lancet<\/em>, 2022).<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">With global attention intensifying, it is clear that protecting antimicrobials cannot be done by the human health sector alone. Human, animal, and environmental health are deeply interconnected\u2014requiring a coordinated approach. Nepal is one of the countries actively embracing this shift, using the&nbsp;<strong>One Health<\/strong>&nbsp;framework to strengthen its AMR response.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>From Concept to Collaboration<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The idea behind One Health is simple yet profound:&nbsp;<strong>the health of humans, animals, and the environment are inseparable<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Pathogens cross these boundaries effortlessly, and antibiotic use in one sector affects all others. In Nepal, the One Health concept is evolving into structured, practical action.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">To coordinate surveillance, improve data sharing, and align policies, Nepal has established several mechanisms:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The\u00a0<strong>One Health Steering Committee<\/strong>, bringing together experts from human, animal, and environmental sectors.<\/li>\n\n\n\n<li>A\u00a0<strong>Joint AMR Surveillance Program<\/strong>, integrating surveillance across hospitals, veterinary centres, and environmental sites\u2014an important step toward institutionalizing One Health.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Addressing AMR requires more than medical expertise; it needs shared data, joint action, and synchronised stewardship across all sectors.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>The Role of the Fleming Fund Country Grant Nepal (FFCGN)<\/strong><\/h1>\n\n\n\n<p class=\"wp-block-paragraph\">Through initiatives supported by FFCGN, multidisciplinary teams across human and animal health laboratories, veterinary centers, and food testing facilities are generating&nbsp;<strong>quality-assured data<\/strong>&nbsp;that guide national decision-making.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>These efforts are:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Strengthening laboratory capacity and surveillance systems<\/li>\n\n\n\n<li>Supporting evidence-based antimicrobial use<\/li>\n\n\n\n<li>Enhancing infection prevention and control across clinical and veterinary sectors<\/li>\n\n\n\n<li>Facilitating policy alignment under a unified One Health framework<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Together with the Government of Nepal and partner institutions, these initiatives are transforming AMR containment from concept to&nbsp;<strong>coordinated, sustainable action<\/strong>.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>What One Health Looks Like in Practice<\/strong><\/h1>\n\n\n\n<p class=\"wp-block-paragraph\">Nepal\u2019s One Health experience shows that success begins with building capacity\u2014from the laboratories that test specimens to the national systems that analyze, share, and apply surveillance data.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><em><strong>Human Health Sector<\/strong><\/em><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Hospitals in Nepal\u2019s AMR surveillance network are now generating actionable laboratory data to support&nbsp;<strong>evidence-based clinical care<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Key activities include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Strengthened\u00a0<strong>Laboratory\u2013Clinician Interface (LCI)<\/strong>\u00a0through interactive workshops.<\/li>\n\n\n\n<li>Improved communication between clinicians, nurses, and lab professionals.<\/li>\n\n\n\n<li>Better specimen collection and faster culture and sensitivity testing.<\/li>\n\n\n\n<li>Standardized reporting formats to improve clarity and decision-making.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">FFCGN has also supported&nbsp;<strong>Antimicrobial Use (AMU) Point Prevalence Surveys (PPS)<\/strong>, revealing overuse of broad-spectrum Watch and Reserve antibiotics, providing crucial data for guideline development.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Strengthened&nbsp;<strong>Infection Prevention and Control (IPC)<\/strong>&nbsp;committees and Antimicrobial Stewardship Programs (AMSPs) rely increasingly on laboratory results to guide action.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">At the national level, surveillance data directly influence:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Treatment guidelines<\/li>\n\n\n\n<li>Diagnostic standards<\/li>\n\n\n\n<li>Policy decisions<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><em><strong>Animal Health Sector<\/strong><\/em><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Poultry farm surveillance across all seven provinces reveals valuable insights into antimicrobial use in food-producing animals.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Findings include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Widespread use of antibiotics without veterinary oversight<\/li>\n\n\n\n<li>Use of banned or high-priority antibiotics such as\u00a0<strong>colistin<\/strong>\u00a0and\u00a0<strong>fluoroquinolones<\/strong><\/li>\n\n\n\n<li>Limited awareness of withdrawal periods<\/li>\n\n\n\n<li>Weak biosecurity practices at farms<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">These insights now drive discussions among policymakers, veterinarians, and farmers to promote:<\/p>\n\n\n\n<ul in:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Good Manufacturing Practices (GMP)<\/li>\n\n\n\n<li>Hygiene and handling<\/li>\n\n\n\n<li>Residue monitoring<\/li>\n<\/ul>\n\n\n\n<p response<\/strong>.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>Building Systems: Integration, Digital Transformation, and Governance<\/strong><\/h1>\n\n\n\n<p class=\"wp-block-paragraph\">Nepal\u2019s One Health progress is not just about data collection; it\u2019s about building systems.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">National AMR governance now includes:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>National Steering Committee for AMR (NSC-AMR)<\/strong><\/li>\n\n\n\n<li><strong>Sectoral AMR\/AMU Technical Working Groups (TWGs)<\/strong><\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">These bodies ensure regular dialogue, joint learning, and coordinated decision-making across human, animal, food, and environmental sectors.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Digital Transformation<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The integration of Laboratory Information Management Systems (LIMS) is modernizing Nepal\u2019s AMR network by:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Streamlining data entry and reporting<\/li>\n\n\n\n<li>Improving accuracy<\/li>\n\n\n\n<li>Reducing manual workload<\/li>\n\n\n\n<li>Accelerating national and global reporting<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">This shift allows laboratories to focus more on testing and interpretation, and less on paperwork.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>Policy and Governance Framework in Nepal<\/strong><\/h1>\n\n\n\n<p class=\"wp-block-paragraph\">Nepal has now established several key national frameworks that anchor the One Health approach:<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>\u2014 National Action Plan on AMR (2024\u20132028)<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Formalizes multisectoral coordination and strengthens governance structures.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>\u2014 National Antimicrobial Treatment Guidelines (2023)<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Integrates cumulative antibiograms and WHO AWaRe classification for rational antibiotic use.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>\u2014 National Essential List of Veterinary Medicines (2023)<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Promotes responsible antibiotic use in animals and aligns with One Health principles.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Together, these frameworks provide a strong foundation for integrated AMR containment.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>Lessons Learned<\/strong><\/h1>\n\n\n\n<p class=\"wp-block-paragraph\">Nepal\u2019s One Health experience highlights that effective AMR control depends on:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Engaged stakeholders<\/strong>\u00a0from clinicians to farmers<\/li>\n\n\n\n<li>Strong\u00a0<strong>laboratory\u2013clinician communication<\/strong><\/li>\n\n\n\n<li>Direct linkage between\u00a0<strong>evidence and action<\/strong><\/li>\n\n\n\n<li>Multisectoral coordination grounded in real-world data<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>A Shared Responsibility<\/strong><\/h1>\n\n\n\n<p class=\"wp-block-paragraph\">As we observe&nbsp;<strong>World Antimicrobial Awareness Week (WAAW) 2025<\/strong>, it is vital to remember that preserving the power of antibiotics is a collective responsibility.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Clinicians and pharmacists<\/strong>\u00a0must prescribe and dispense responsibly.<\/li>\n\n\n\n<li><strong>Farmers and veterinarians<\/strong>\u00a0must avoid unnecessary antibiotic use and maintain good hygiene and biosecurity.<\/li>\n\n\n\n<li><strong>Everyone<\/strong>\u00a0must avoid self-medication, complete prescribed doses, and practice infection prevention\u2014including hand hygiene.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Nepal\u2019s growing success with the One Health approach demonstrates that collaboration, evidence sharing, and innovation can transform AMR containment from concept to impact.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">By building on these foundations,
