His professional interests include trauma management, rural orthopedic care, and improving surgical services in underserved regions through research and academic contributions.

When someone is injured, attention is usually focused on the immediate pain and the need for care. Yet, in remote villages, even a simple fracture can quickly become an economic crisis for an entire family.
For many rural families, a broken bone forces them to lose the daily work they rely on to survive. The financial problems caused by an injury often last long after the body starts to heal.

The Vulnerability of the Breadwinner
In places like Bajura, people depend on hard physical work to survive. Most families do subsistence farming, raise livestock, work for daily wages, or carry loads, so physical strength is their main way to earn money.
If the main earner breaks a bone, the family’s income can suddenly stops.
Unlike formal jobs, rural areas do not offer paid leave or alternative employment. When the main earner cannot work, the family income often disappears. Serious injuries require ongoing care, sometimes forcing another family member to stop working or miss school, which makes the situation even harder financially.
In subsistence farming, missing an important time, such as planting or harvest, can create more than a short-term problem. It can cause food shortages and hardship for months, or even a whole year, affecting the entire family.

Beyond the Hospital Bill
A broken bone costs more than just the obvious expenses like X-rays, casts or surgeries. For people in remote rural areas, hidden indirect costs often become the hardest challenge.
Distance and transport create the first problem. In areas with poor roads access, families must carry injured people on stretchers, hire vehicles, or pay for expensive public transport. Many families use up months of savings just to reach the hospital.
Patients rarely travel alone. Extra costs for fooding, lodging, and necessities for caregivers in hospital towns can add up quickly, sometimes exceeding treatment costs.
Even though the government health insurance and public hospital support make healthcare more accessible, families still pay substantial out-of-pocket costs for procedures and medicines. They often take out high-interest loans or sell important assets, such as livestock or land, to cover these costs. This debt can last for years, turning a short-term injury into a long-term financial problem.

The Orthopaedic Surgeon Restoring Lives and Livelihoods
In tough economic conditions, an orthopaedic surgeon does much more than just fix broken bones. In rural communities, the surgeon often helps protect both families’ health and financial stability.
By providing timely, effective, and local orthopaedic care, the surgeon not only heals bones but also restores hope and stability for families facing hardship.

Impact of Local Orthopaedic Care

Reduces Financial Strain: Local treatment for trauma and complex wounds eliminates the expense of travel and accommodation to distant cities.

Accelerates Recovery: Modern methods stabilize injuries more quickly, enabling main earners to return to physical work sooner and protect family income.

Prevents Disability: Timely care avoids poor bone healing, lasting pain, and permanent deformity that cause lifetime income loss.

Ensures Affordability: Rural surgeons optimize resources and help patients access government funding to remove financial barriers to surgery.

A Call for Rural Healthcare Investments.
In rural Nepal, a broken bone is far more than a simple injury. It can threaten a family’s whole livelihood and disrupt their health and economic security.
To truly reduce this burden, we must now act. Invest in district orthopaedic care and make services accessible, so families can recover quickly and avoid falling into poverty. Prioritize rural health care to restore livelihoods and break the cycle of injury and poverty.

Dr. Shirish ShresthaHe is a Consultant Orthopedic Surgeon currently serving at a district hospital in Bajura, Nepal, where he is dedicated to delivering quality musculoskeletal care in a resource-limited setting.
