She is leading the Pelvic Floor Center, which aims to fill the gap in current healthcare through comprehensive , accessible and evidence based care for individuals with pelvic floor disorders. She is the Chief of Education and Training Division, Dhulikhel Hospital.<\/em><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Pregnancy and postpartum dysfunction bring a broad set of symptoms in women resulting from abnormal function of the pelvic floor musculature, including muscles, nerves, and connective tissues. Lifestyle, medical and surgical conditions also affect pelvic floor function. The disordered function corresponds to either increase activity (hypertonicity) or diminished activity (hypotonicity) or inappropriate coordination of the pelvic floor muscles. Pelvic floor disorders in female include pelvic organ prolapse, urinary and fecal incontinence, chronic pelvic pain and sexual dysfunctions. The symptoms can present individually however are often interrelated. The disorders are highly prevalent, but few are underdiagnosed and undertreated. In a country like Nepal limited awareness, lack of specialized services and cultural stigma prevent women from seeking timely care. These disorders are often treated in isolation however, optimal management requires a holistic, multidisciplinary approach.<br>Dhulikhel Hospital inaugurated Nepal&#8217;s first multidisciplinary Pelvic Floor Center on 28th July 2026. This center was officially inaugurated by Dr. Ram Kantha Makaju Shrestha, founder and executive director of Dhulikhel Hospital and Prof. Dr. Raymond Peter Hodgson, Australian gynecologist from University of New South Wales in presence of the president of Nepal Society of Obstetrics and Gynecology and other Urogynecologists and Gynecologists, colorectal surgeons, physiotherapists from different hospitals of Nepal.<br>It is a matter of immense pride in letting you know that I will be leading this Pelvic Floor Center, which aims to fill the gap in current healthcare through comprehensive, accessible and evidence-based care for individuals with pelvic floor disorders. The individuals experiencing pelvic organs prolapse, urinary and fecal incontinence, people experiencing chronic pelvic pain and people facing sexual dysfunction will be treated in this center. The postpartum women who can develop childbirth related pelvic floor dysfunction like, pregnant women and postpartum women, women who experienced difficult and traumatic vaginal birth, women who had anal sphincter injury and women who experienced pelvic floor weakness following childbirth can be managed here. Aging adults like women of perimenopausal age group and elderly people can also develop pelvic floor disorders. They can thus benefit from this center through rehabilitation physiotherapy. Patients with chronic disorders effecting pelvic health, e.g. COPD, constipation, or any condition that persistently increase intraabdominal pressure (heavy weightlifters are also candidates needing preventive care). Post surgical rehabilitation to prevent pelvic floor dysfunction is needed to all individual following all gynecological procedures in women and prostate surgery in men.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Expected outcome<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>In this center we expect patients to experience high patient satisfaction due to coordinated, respectful and specialized care under one roof. They will properly be diagnosed first and will be provided with different conservative, rehabilitative and needed definitive treatments, reducing unnecessary interventions. Both over and undertreatment will be avoided as this is extremely harmful to the patients. This is then expected to reduce complications related to pelvic floor disorders and untreated conditions.<\/li>\n\n\n\n<li>This center takes responsibility for preventing pelvic floor disorders by enhancing health care capacity in pelvic health. They will work with obstetricians, midwives and nurses to improve pelvic floor function following pregnancy and delivery. Perimenopausal and elderly women are also susceptible for pelvic floor dysfunction. Identification of vulnerable groups becomes a priority of this center and will work for prevention and rehabilitation of different pelvic floor disorders.<\/li>\n\n\n\n<li>Improved perineal hygiene and nutrition are important components for pelvic floor function. Though they are not directly related, indirectly it can reduce aggravating factors and support pelvic tissues. Nutritionists can help patients maintain healthy body weight, prevent constipation and suggest maintaining adequate body hydration preventing dehydration and excessive hydration that worsen the urinary symptoms. Nutritionists can also help suggest avoiding food and drink that irritates pelvic organs.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The establishment of Pelvic Floor Center reflects Dhulikhel Hospital&#8217;s ongoing commitment in expanding specialized healthcare services and addressing unmet needs in pelvic health.
